MAGNUS Health Forms
All health forms were due on August 1 and must be completed before students can begin on-campus learning. Please be sure that you have submitted the required health forms to MAGNUS Health. If you have any questions regarding these forms, please reach out to Erika Herridge.

To complete the forms:
1. Visit www.hockaday.org and click the green Login button in the top right corner of the homepage
2. Input your Blackbaud Username and click “Login”
3. Click the MAGNUS | Medical Portal on the Parent Resource Board
4. Click on the orange MAGNUS Medical icon at the top of the page.
5. Click the orange “Complete Now” button to open your daughter’s tracker and follow instructions to complete the forms.
