Medical Forms for 2023-24 School Year due by July 1
- Student health and medical information is collected through the Magnus Health Portal. This secure system meets HIPAA privacy requirements and provides critical health information to healthcare providers in the event of an emergency and is due July 1.
- The online Student Medical Record (SMR) must be updated for every student every year for the next school year. This includes updating the vital health record, giving OTC permissions, etc. A Medical Certificate is required for Grades K, 1, 3, 5, 7, 9, 11, and all NEW students. For 7th grade, proof that your child has received a Tdap booster and the Meningitis vaccine is required. Please print the form and take it to the doctor or any clinic that does back-to-school physicals.
The 2023-2024 Medical Certificate is available on the Hockaday Resource Board and here for parents to fill out and complete if required for your student by Saturday, July 1.
