Student Health and Medical Information
In preparation for the 2026 – 2027 School Year, it is time to collect student health and medical information through the Magnus Health Portal. This secure system meets all HIPAA privacy requirements and provides critical health information to healthcare providers in the event of an emergency.
Please complete the Student Medical Record (SMR) by Wednesday, July 1, 2026, instructions below. This enrollment requirement must be completed and approved by the nurse before your daughter can attend pre-season athletic practices or start school.
- Medical Certificate – Required for Grades PK, K, 1, 3, 5, 7, 9, 11 and all NEW students. You must print this MEDICAL CERTIFICATE and take it to the doctor to complete and sign.
